The proposed prescription should identify the selected strength and the actual supplied product. The public medication quote is $90 for a tube described as a three-month supply. Dividing by three gives $30 per month for the medicine alone. Confirm the actual quantity, refill timing and instructions for the prescription offered. Do not assume every competing bottle described as “monthly” contains an equivalent amount.[source]
The page says medicines are shipped and billed according to the selected plan. Read that plan before treating the supply estimate as a guarantee of monthly payments. A change in the prescription can also change which tube or refill arrangement is appropriate.
The lower number is not the complete bill
The acne FAQ lists a $40 consultation charge and a $3 support fee when an order is processed in a month. Insurance may affect the acne medication cost where accepted; Nurx says it does not accept insurance for anti-aging care. These conditions prevent the advertised “as low as $0” medication figure from becoming a general free-treatment claim.[source]

How this differs from a named custom cream
A single-active tretinoin product and a personalized blend containing tretinoin answer different formulation questions. The blend may include other actives that are useful to discuss, but more ingredients do not automatically establish a better result. Ask why each ingredient is proposed and how previous irritation affects the choice.
For a concrete alternative, compare the Dermatica formula with the actual Nurx prescription, using active identity, strength, base, quantity and follow-up. The service names alone are insufficient to establish equivalence.
